We are excited you have chosen us for your child’s dental care needs. Please take a few minutes to complete our online registration forms that will be directly sent to you via text to the phone number you provided when scheduling, or to the email address provided. If you are unable to complete the forms before your visit, please plan to arrive 10-15 minutes early for your appointment so that you can complete them in our office. We have iPads available if needed.
